I noticed an issue with MacOS voiceover where it does not announce the "unselected" state for web radio buttons. (Interestingly, notice that iOS Voiceover Safari does announce "unchecked".) - You can also reproduce this by testing MacOS Voiceover on the radio buttons on the same website.
